NOBLES COUNTY FAMILY SERVICES AGENCY: 

An agency that provides case management services to disabled persons which can be defined as identifying the need for, planning, seeking out, authorizing, and coordinating services to disabled persons.  Some of the residential support services listed in this section as well as additional services such as respite care, homemaker or chore can be accessed through this agency.

SEMI-INDEPENDENT LIVING SERVICES: 

Direct services offered to persons 18 years of age or older who have diagnosis of mental retardation or a related condition.  These services can be provided in their own home, parental home, etc. on less than a 24-hour per day basis.  Program staff offer training, counseling, instruction, supervision, and assistance to clients in the areas of meal planning, and preparation, money management and budgeting, shopping, health care, laundry, personal appearance and hygiene, home maintenance, social and transportation skills plus other areas as determined by the person and/or their family members, county social workers, and SILS advisors.  Services need to be accessed through the County Social Service Agency.

WAIVERED SERVICES: 

A residential support service for persons with a diagnosis of mental retardation or related condition that can be provided either in the personÕs family home, as in-home family support, or in a living situation independent of the family, as supported living services.  Assistance, training and supervision and monitoring can be provided in areas of self-care, sensory and motor development, interpersonal skills, communication, socialization, care, leisure, and recreation, money management and household chores. Services need to be accessed through the County Social Service Agency.

SOUTHWESTERN CENTER FOR INDEPENDENT LIVING (SWCIL): 

Provides independent living services to the individual with disabilities.  Training can be provided in the following areas:  self-care, housekeeping, meal planning and preparation, clothing care, budget and checking account management, shopping skills, etc.

RIDGEWOOD:

A residential service that provides 24 hour care, supervision, food, lodging, rehabilitation, training, education, habilitation and treatment for 15 persons with mental retardation or related conditions.  The facility offers a homelike atmosphere that is a barrier free for persons who use a wheelchair.  Support and supervision is provided by shift staff, including some nursing personnel. Services need to be accessed through the County Social Service Agency.

WORTHINGTON HOUSING AND REDEVELOPMENT AUTHORITY: 

This agency will assist people with obtaining low-income housing.  Qualified tenants may also participate in the FAMILY SELF-SUFFICIENCY PROGRAM, which provides support in striving toward a goal of non-use of public assistance.
