Resource Directory Overview

According to Minnesota Statue 120.17 Sub #3A, the Individual Education Plan (IEP) will, by grade nine or at age 14, address the studentŐs need to develop the skills to live and work as independently as possible in the community.  This includes the studentŐs needs for transition from school to post secondary education and training, employment, and community living.

 

This reference guide was developed in the hope that students and their families who are involved in the transition process will find some direction and support through the information provided in this guide.  Since the purpose of this guide is to provide information that will ensure a smooth transition from school to community living, the names of agencies/organizations that will be an integral part of the process are included along with a summary of that agency/organizationŐs services.

 

As the guide is read, students and parents should be aware that not all of the agencies/organizations listed will be applicable to every individualŐs situation and a careful investigation of potential agency/organization use is necessary to ensure the maximum utilization of appropriate resources.
